Collapsing and ExpandingField PHP FormMail Generator

 
Here's a highly sophisticated formmail
generator. This one is very capable and
seems to include fileds that collapse and
expand as needed.

In other words, if a group of fields are
not applicable, they will collapse or if
they are applicable, they will expand.

This formmail program is written in PHP
and makes use of JavaScript to take care
of the expanding and collapsing fields.
